I would say get a shirt similar in size paint the image onto the front (it looks like a flower) or if you have another picture of the image its self (not covered) you could print it out onto a transfer and iron it on.
Then in the surrounding area draw with a pencil a pattern similar to a cyclone fence and cut out the inside spaces. It looks like there's a solid strip down the front. So leave that part alone. The bottom should be pretty simple to do the easiest way would be to cut out a triangle trace around it at the bottom and cut. This will ensure that all of the points will be the same.
