I actually used fleece on this because I didn't have any different fabric. That seemed to make everything so much harder. ): If you need some help, feel free to comment. I also don't have pictures of this because it was done late at night. But I have MSPaint pictures.

Figure out how long and how wide you'd like your bow to be. Take these measurements and multiply by 2. Cut out a piece of fabric with the new doubled measurements.
Fold the fabric over "hotdog" style. Sew across the red line.
Turn it inside out. Move the fabric so the seam line is in the middle.
Fold it over so that the seam line is on the outside. Sew along the red line.
Now turn it inside out. Bring the newest seam line to the middle.
Pinch down in the middle and secure. You can sew it down, rubber band it down, whatever you please. You will be hiding it anyway. I used an old hair tie. The next step shows the hair tie securing.
How I secured the pinch.
Cut out a small piece, enough to cover up the middle. Slip it through the elastic headband. Or, if you have just plain elastic, you can attach it later.
This is what mine looked like.
TAAADAAAA. You're done. (:
