This was a proto-type that had turned out quite well. So, that's why there's not photos for most of the steps. Sorry, chaps.

Get your tape measure, and make sure it's the flexie kind and measure either halfway around your neck or 3/4 around your head.
Cut 2 pieces of the wide lace, the contrast lace, and the holey-windy lace (pictured) to the measurement you just took.
Pin and sew the gathered part of the lace together, thusly.
Sew the contrast lace to the gathered lace on either side, as shown.
Entwine the ribbon through the wholes in the ribbon through the holes in the lace. Pictured:
Sew the entwined ribbon to the other lace. So
Sew the ends down on both sides. Like this:
"Let's give them a screwing they'll never forget!" You must decide what frequency your screws will be. I chose every time the ribbon went under the face. Be warned though, too many or too few will look very odd.
Inserting a screw: Choose the placement of the screw. Find the coresponding spot on the back. (This next part is sorta hard to explain, but Ill try. Give me a warning if it makes no sense and I may make a video on it.) Cut through the lace (I don't suggest cutting the ribbon)JUST enough to get the screw through. TWIST the screw trough. It's kinds pushing and twisting, but if you push at the beginning, it will not go well
Apply the nut in the usual, and obvious fashion.
Season screws to taste. I ended up with 9 screws (screwing 9 times in an afternoon is quite an accomplishment)
Attach the extra ribbon to either both ends of the ruff/headress.
