To make this book using a small cereal box I followed the instructions on the book Eco Books by Terry Taylor. This book is available for purchase from Amazon. I checked out a copy of this book at my Library. Note: to get a better view of the images click on each image.

Dismantle the cereal box. Trim the flaps using a paper cutter. You will use the front and back and one of the sides. The smallest side will be the spine of the book.
Make a punch template out of paper using the smaller side which will be used as the spine. Draw 4 parallel lines and 4 horizontal lines on this punch template. You will using the awl to punch small holes at each intersection.
Position the punch template you created on the short side of the box.
Place the box on top of the mouse pad and punch through each intersection using the awl. You can use clips to keep the template from shifting.
Use a variety of papers and cut them to measure twice the width of the cover and the height minus 1/4 inch in each direction. Make 4 stacks containing 6 pieces of paper and fold in half. Crease well using a bone folder. These are the signatures.
Use the punch template you used to punch the holes on the shorter side of the box to mark 4 dots on the middle crease of one of the sheets. You will use this as the template to punch the holes on the paper using the awl.
The yellow pages of the phone book works well to punch the holes on the six pages of paper. Punch all 4 stacks of paper which contain 6 pieces of paper that have been folded in half.
Cut a piece of linen thread which measures about 8 times the height of the book. To sew the 4 stacks of paper (also called signatures) you will be using the long stitch binding technique. At the beginning leave a tail.
As you sew the signatures to the spine, you should have two long stitches on the middle of the sheets.
As you sew the signatures, the stitches on the outside look longer. Tighten the thread as you sew.
The 4 lines of stitches on the outside of the box.
Notice that the holes on the top and bottom section are used to connect each signature and will look like two short lines. Once all the signatures are sewn tie a square not to the beginning tail.
