Simply cut out the straight shape of a tube top(the red lines) from a men or woman's dress shirt. Sew up the sides to fit you a little better if the shirt is too big. Make sure it isn't too fitted though. Video tutorial: http://www.youtube.com/watch?v=aIMa2j_KbM8
Cut the shirt shorter(from the top edge) if it's too long. Remember that about 5 inches will be overlapping the top so you need to leave some extra at the top. You can hem the top edge, I didn't on mine. REMEMBER to hem the top hem outwards, because the inside of the shirt will be facing outwards with the flaps. DO NOT sew the opening together when you hem it, just sew the hem when the tube top is open.
To put it on, start buttoning from 1 or 2 buttons from the top, this new top button should be buttoned at the top of your bust. Button the rest of the buttons and take the extra fabric on the top and fold it over the front. Tutorial for bow belt: http://www.youtube.com/watch?v=1b2BoocVPb8
